I'm not talking extreme in the shovel racing sense of the word, but extreme in the literal sense—extreme (adj) going to great or exaggerated lengths; exceeding the ordinary or expected. There are not many places where people surf in board shorts in the summer and 5 mil wetsuits in the winter.
